Formal evaluation of stage one of the Health Care Homes programme will be undertaken by the government. The evaluation will include reviewing the financial modelling to help to determine how many patients are eligible, how many patients will enrol, and their tier levels.

PHNs and Health Care Home practices may wish to do their own evaluations and improvement activities.

An evaluation fact sheet is being prepared to provide further clarification on the types of data being collected and associated timeframe. It is known that patient out-of-pocket costs for Health Care Home services will be monitored during the stage one trial.

Patient Reported Measures (PREM & PROM)

Patient Reported Experience Measures (PREMs) and Patient Reported Outcome Measures (PROMs)[1] are effective for understanding the quality of care. In 2017, the OECD Health Ministers said that PREMs and PROMs will " better equip countries with data that reflect what matters for patients"[2] .
