Care coordination is done across multiple settings and includes communication with the patient and members of the care team.
The key tasks of care coordination are:
- Determining and updating care coordination needs.
- Facilitating transitions.
The activities are driven by the patient's care plan.
Care coordination need not be restricted to medical services. Taking a holistic approach, it can include services, activities and organisations to meet patients' health and well-being needs. One model is social prescription, which requires a coordinator (usually not clinical) who has an in-depth understanding of the community's resources, needs, and strengths.
